Toshiba Medical’s Fully Integrated Model-Based Iterative CT Reconstruction Receives FDA Clearance

FIRST Technology Improves Image Quality and Lowers Dose for Safer Exams


CHICAGO, Nov. 28, 2016

Model-based iterative reconstruction is now fast enough for customers' clinical needs as Toshiba Medical's Forward projected model-based Iterative Reconstruction SoluTion (FIRST) received FDA clearance. FIRST improves high contrast spatial resolution while reducing radiation dose up to 85.3 percent as compared to filtered back projection and drastically cuts the time needed for model-based computed tomography (CT) image reconstruction.

"FIRST produces images that are natural-appearing and processes them fast enough for routine clinical work," said Dr. Marcus Chen, director, Cardiovascular CT, National Heart, Lung and Blood Institute, National Institutes of Health, who was part of the beta testing for FIRST. "With FIRST, we can improve image quality while lowering radiation dose." NHLBI has a research agreement with Toshiba Medical to help develop and test new technologies for CT.

FIRST can improve high-contrast spatial resolution by up to 129 percent (2.29 times) as compared to filtered back projection for body imaging while reducing radiation dose. The easy-to-use technology can be integrated into scan protocols adapting to the individual patient and clinical task at hand, requiring as little as three minutes for volumetric reconstructions.

Cardiac imaging with MRI is one of the most challenging examinations. The cS Edition achieves significant improvements in the efficiency of cardiac examinations, while acquiring high-resolution images during daily clinical practice with the use of the latest intelligent solutions. This system meets a diverse clinical requirement while expanding the range of day-to-day examinations.

Development background

The Vantage Titan system has been developed with the concept to perform advanced examinations while improving patient comfort with user-friendly technologies. Toshiba's Vantage Titan maintains robust image quality even with the wide-bore concept. The system has been recognized worldwide as a wide-bore MRI system that permits a high image quality in wide scan range. The cS Edition is equipped with a number of cutting-edge technologies for more advanced and efficient examinations. Features of the new product

1. High-resolution images through enhanced flexibility

In the cS Edition, we have expanded clinical applications with a "New Sequence Design Environment" which enables flexible parameter change with a diverse pulse control, achieves further image quality enhancement.

For example, in Flow Sensitive Black Blood (FSBB) sequence, an independent MPG-pulse can control the strength on the phase, frequency, and slice direction. In addition, it is now possible to perform weighted processing on phase images while displaying the original phase image. As a result, finer details of narrowing blood vessels and variations in susceptibility can be easily determined.

Now the system supports Multi b-value diffusion with the cS Edition while implementing automatic calculation of isotropic images for each b-value. Beyond offering the freedom to choose different b-values, the operator can utilize the Multi b-values for cDWI based upon the clinical region of interest.

When utilizing FFE3D sequence, flexibility is increased by linking multiple parameters such as bandwidths, sampling rates, resolution, and image cross-section. The sequence increases the SNR and T1 contrast to improve diagnostic quality.

2. A system with next-generation assisted positioning to make difficult scans routine

Today's MRI must meet growing demands for more daily examinations and a wider range of adaptive body regions scanned. It is very important to be able to perform highly-reproducible scans in a short period of time without being overly dependent on operator skill or experience. The cS Edition incorporates EasyTech, advanced scanning plane positioning assistance functions: NeuroLine+ where OM and AC-PC cross-sections have been added to NeuroLine, as well as the SpineLine for spinal positioning. These features reduce operator workload and save time in complex positioning tasks.

The leading cause of sudden death has been linked to heart disease. Cardiac MR(CMR) is anticipated to be used for general medical screening in hope to prevent sudden death. However, currently there are two limitations for CMR resulting in remaining unfamiliar with daily examinations. CMR requires a higher skill set and knowledge-base to examine a complex anatomical region. Furthermore, a variety of specific sequences need to be performed with specific views of the heart thus extending examination time. The cS Edition is incorporated with EasyTech Cardiac which consists of CardioLine+ and SUREVOI™. In addition to set six basic cardiac planes, further eight-planes setting become available with CardioLine+: right ventricle, valves and outflow tract as defined by the protocol of the Society for Cardiovascular Magnetic Resonance (SCMR). Conventionally, patients were asked to hold their breath multiple times to acquire images along the target planes. It is now possible to acquire 14 cross-sections with a single breath hold. This reduces exam time and alleviates excessive breath hold times and stress on the patient.

The system is also equipped with SUREVOI Cardiac to provide a higher level of convenience in whole CMR examinations. SUREVOI Cardiac determines the position of the heart by using anatomy recognition technology. By assisting in the various scanning alignments, from the patient table position settings to the MRCA diaphragm gating probe settings, our EasyTech Cardiac technology reduces the operating time that typically represents 43% of CMR examination time down to 9%.

With the goal of making CMR examinations more readily available, the cS Edition makes complex cardiac examinations now routine.

3. Providing new advanced solutions for customers' clinical requirements

Customers' clinical requirements become more diverse and more advanced from day to day. The cS Edition is newly equipped with many clinical applications designed to meet these advanced requirements. One of these clinical features is UTE (Ultrashort TE), which promises to eliminate radiation exposure in lung field MRI examinations. UTE enables visualization of regions that had been difficult to depict with conventional MRI systems by imaging tissues with extremely short T2* values. UTE has already been praised at many of our Vantage Titan 3T sites, with customers saying, "This technology expands the scope of MRI clinical applications within various body regions."

State-of-the-art post-processing clinical applications can be achieved through the Vitrea™ and in collaboration with Olea Medical SA (acquired by Toshiba Medical Systems in October 2015), such as IVIM analysis to visualize intracellular perfusion in DWI. Additionally, Wall Motion Tracking to evaluate myocardium strain from Cine imaging in support of a diversity of clinical requirements.

New technology to project expansive virtual reality images inside the magnet bore

Reducing patient anxiety caused by the confined MRI examination space

Toshiba Corporation and Toshiba Medical Systems have developed a technology for displaying expansive virtual reality images inside the bore*1 of MRI systems. High-reality images are projected onto a dome-shaped screen (dome screen) in the bore to take the patient's attention away from the actual examination space. This new technology is a reference exhibit*2 at the 101st Scientific Assembly and Annual Meeting of the Radiological Society of North America (RSNA 2015, November 29 to December 4, Chicago).

Development Background and Conventional Technologies

To reduce the patient anxiety caused by the confined examination space and noise during MRI examinations, Toshiba Corporation and Toshiba Medical Systems have previously developed a 71-cm large-aperture open-bore MRI system and the PianissimoTM quiet scan technology and released them in the market. The newly developed projection technology further improves patient comfort and reduces the patient stress caused by the MRI examination environment.

Features of the Technique

A semi-transparent dome screen that is moved in synchronization with the patient table*3 is installed inside the bore, and images are projected onto the dome screen and bore cover from a projector, which is installed behind the MRI system, and whose location is unaffected by the magnetic field. The images are reflected by a mirror installed on the patient table and can then be viewed by the patient, providing a visual space that helps take the patient's attention away from the actual examination space (image 1).

The core technologies used in the in-vehicle head-up display*4 and ultra-high-presence head-mounted display*5 systems developed by Toshiba Corporation are incorporated in the dome screen installed in the bore. In addition to the center field,*6 where the colors and shapes of objects are processed, image stimulus is applied to the peripheral field,*7 where spatial depth and width and object movements are processed, achieving high-reality images with a view angle of as wide as 60° or more. The images reflected by the mirror appear as if displayed on a screen much further away than the actual bore cover, providing an expansive virtual space for the patient in the bore (image 2).

Before starting the examination, the dome screen (on which images are displayed) is located at the entrance of the bore. Since the tunnel cannot be seen, patient anxiety related to the examination space is reduced (image 3). When the examination is started, the dome screen is moved into the bore in synchronization with patient table movement, allowing the patient to view images continuously without changing the image size even though during the table movement, suppressing anxiety related to the confined space inside the bore. The high-presence images reflected by the mirror and projected onto the bore cover then come into view, providing the patient with an expansive and bright virtual space.

Future Prospects

Together with our noise reduction technology, this new technology will help achieve a comfortable patient environment that eases patient anxiety in the bore. We will strive for the early commercial release of this feature.

World-Class Teaching Hospital and Research Center Installs First 640-Slice Volume CT System in Oregon

Oregon Health & Science University now offers patients one of the most advanced CT imaging systems available with the installation of Toshiba’s Aquilion™ ONE ViSION Edition. OHSU, a nationally prominent research university and Oregon’s only public academic medical center, acquired the new system to help improve its diagnostic capabilities for trauma, cardiac, stroke and pediatric patients. Adding the first 640-slice system in the state of Oregon allows OHSU to deliver better patient care and maintain its position as one of the leading health care institutions in the United States.

The Aquilion ONE ViSION Edition and its PUREViSION Detector provide unique features for a complete CT solution. The system produces high-quality images and fast workflow with 640* of the industry's thinnest slices (0.5 mm) and a gantry rotation of 0.275 seconds. Its 16 cm of coverage is capable of imaging entire organs, like the heart and brain, in a single rotation for faster and more accurate cardiac and stroke exams. Patients are also more comfortable and safer, as the system features a large 78-cm bore and iterative dose reconstruction software, AIDR 3D, that significantly reduces radiation dose compared with conventional scanning.

To meet both clinical and research needs of MR customers, Toshiba America Medical Systems, Inc. has implemented software and hardware upgrades for the Vantage Titan™ 3T MR imaging system. Available both for new Titan 3T MR systems and for existing installations, these upgrades improve image quality and workflow so those in both the clinical and research settings have access to the highest levels of performance and information to provide the best possible care.

The upgrades feature several enhancements:

  • Saturn Gradient: This entirely new gradient design option provides increased performance and image stability for maximum quality.
  • fMRI Enhancements: With Pianissim™ noise reduction technology, Titan 3T MR systems deliver the quietest fMRI in the industry, ensuring a patient-friendly environment.
  • Diffusion Tensor Imaging: With the benefit of the new gradient design and the Pianissimo noise reduction technology, Toshiba delivers outstanding performance in a patient-friendly environment.
  • Isotropic Imaging: Toshiba's mVox 3D isotropic imaging makes high-quality MPR 3D post-processing possible in any plane.
  • Arterial Spin Labeling: Time-SLIP labeling provides improved visualization of cerebral vascular circulation without the use of contrast media.
  • 16-Channel Flex Medium and Large Coils: These multi-purpose coils improve signal-to-noise ratio for better high-resolution imaging of a variety of anatomical regions.
  • Updated M-Power Interface: This user-friendly interface now includes more flexibility in parameters and new sequences to offer operators better control and faster access to crucial information.
